Step 1: Print this petition only on one sheet of 8 1/2 x 11 letter size paper.

Step 2:  One to four registered Dana Point voters may complete a box by printing their name and residence address on the petition and     by  signing their name in the space provided (no P.O. Box addresses).  Signatures are only valid if the address matches their voter registration records.

Step 3: In the box "DECLARATION OF CIRCULATOR", you must fill in the blank areas in your own handwriting.  Follow the instructions carefully to avoid rejection of the petition.

Only registered voters who are registered in Dana Point can sign the petition.
Only those eligible to vote in California who reside in Dana Point can circulate the petition.
